weight, blood pressure, blood sugars;
4) Secure messaging with an assigned nurse case manager;
5) Alink to a member maintained personal health record;
6) Targeted access to a health library;
Disease Management programs target individuals with chronic medical illnesses through the application of evidence-based care, self-care support, care coordination and leverage of community services.Standard approaches to disease management have included call coaching and direct mail pieces but an internet based program offers an innovative way to engage members where and when its convenient for them. said Dr. Jill Sumfest, CMO, Preferred Health Systems.
